Pays for dentist visits

Companion Life's Dental by Design plan covers cleanings, exams, fillings, crowns, and more, with no waiting periods. Employees can see any dentist, and in-network dentists charge agreed-upon discounted fees. Two options set the annual maximum at $1,000 or $2,000 per covered person, and the high option adds orthodontia for dependent children under 19.

What this plan includes

• Cleanings, exams, and X-rays are covered at 100% with no waiting period, and preventive care does not count against the annual maximum
• Fillings, extractions, and other basic services pay 80%; crowns, bridges, dentures, and implants pay 50%
• $50 calendar-year deductible per person, up to 3 per family, applying only to basic and major services
• Choose a $1,000 or $2,000 annual maximum per covered person
• The high option adds orthodontia at 50% up to a $1,000 lifetime maximum for dependent children under 19
• Covers employee only, employee and spouse, employee and children, or employee and family
